South East Community Care Access Centre
The South East Community Care Access Centre (CCAC) is one of 14 local CCAS across Ontario that connect people with home and community-based health care, personal support, long-term care homes and other community support services. CCACs can also help connect people in need of a primary care provider with physicians or nurse practitioners.
The South East CCAC’s staff includes nurses and other health professionals called case managers. Case managers are based at KGH and work collaboratively with nurses and physicians to help clients and families plan for the necessary care services to support people upon discharge from hospital. These services might include specialized nursing services, therapy services or personal care (assistance with bathing, dressing).
The South East CCAC maintains an information database containing over 6,000 records of health and community support services and can help with information or service requests. The South East CCAC Access Team is available from 8 a.m. to 8 p.m. seven days a week by calling 310-CCAC (310-2222; no area code is required). Anyone can ask for services from the CCAC.
