TITLE: Nurse Practitioner- Primary Health Care (NP-PHC) 
           1 Full-Time position

POSITION SUMMARY:
Reporting to the Director of Emergency Services, the role of the NP-PHC is to:
 Coordinate, provide and evaluate holistic patient care for patients registered in the Emergency Department with emphasis on diagnosis and management of acute illness in the emergency department. Where appropriate, health promotion and illness prevention opportunities can be realized.
 Provide leadership to promote program development, process improvements and the enhancement of professional standards
 Act as a clinical resource for nurses and other Regulated Health Care Professionals
 Works collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of care
 Coordinates internal/external referrals (follow up) to facilitate best clinical practice as required
 Articulates and demonstrates the integration of the NP-PHC role through research, education and consultation

RESPONSIBILITIES:
 Improving patient outcomes along the continuum of care through the facilitation of evidence-based or best practice guidelines, by intradisciplinary and interdisciplinary collaboration, by participation in research, by the provision of leadership and through education of patients, staff and others.
 A leadership role in the development, implementation and evaluation of relevant procedures, policies, guidelines and standards of care that support patients and their families.
 Work collaboratively with the Emergency physicians in the follow up of Laboratory and Diagnostic imaging results to ensure quality care.
 A leadership role in teaching Medical residents and nurses in the Emergency Department.
 Participates in Emergency Medicine Rounds.

QUALIFICATIONS:
 Eligible for registration with the College of Nurses of Ontario
 Nurse Practitioner- Primary Health Care certificated required (including RN(EC))
 Advanced Cardiac Life Support certification preferred
 Pediatric Advanced Life Support certification preferred
 Trauma Nursing Core Course certification preferred
 Three to five years of recent clinical experience as a Nurse Practitioner- Primary Health Care
 Experience in emergency nursing an asset
 Demonstrated skills in leadership, communication and the process of change
 Presentation, public relations, community education/outreach skills
 Demonstrated excellence in communication, team building, and the ability to initiate and model positive change
 Demonstrated ability to collaborate with other members of the interdisciplinary team
 Demonstrated knowledge and ability in the areas of clinical practice, program development and implementation, and education.
 Proven ability to attend work regularly.
 Satisfactory criminal reference check required.
